Community Outreach

Community Health Workers (CHW) are trained public health professionals who build trusted relationships with patients to connect them with health and social services in the community. CHWs support patients in achieving their health goals by addressing their unique needs outside of the traditional clinical setting.

Care Coordination and Navigation - referrals to healthcare providers, help scheduling and preparing for appointments, and connecting patients with needed services to ensure smooth access to care.

Patient Education and Empowerment - increasing patients’ understanding of their health conditions and promoting healthy lifestyles through education and wellness support.

Social and Emotional Support - Addresses the broader needs that impact health, including mental health care and assistance.
